Procedures

The district will accept community use requests for facility usage for the upcoming school year beginning June 15th (if the 15th falls on a Friday, Saturday or Sunday, sign-ups will be on Monday). Community use is on a first come first served basis.

2025-2026 School Year – Monday, June 16, 2025
2026-2027 School Year - Monday, June 15, 2026
2027-2028 School Year - Tuesday, June 15, 2027

All Applications for Facility Usage terminate at the end of the current school year.

Use of district high school auditoriums.

Use of district fields and use of district parking lots.

All applications for facility usage and auditorium usage terminate at the end of the current school year.

Insurance for Facility Usage

All Non-Fort Zumwalt School District groups wanting to utilize buildings in the Fort Zumwalt School District must provide proof of insurance.

The district accepts the following three types of insurance:

A Certificate of Insurance (COI). This is the preferred proof of insurance by the district and the district’s insurance company. The following is required for the district to accept a COI:
1. The Fort Zumwalt School District must be the certificate holder with the 555 East Terra Lane, O’Fallon, MO 63366 address.
2. The Fort Zumwalt School District must be listed as an additional insured in regards to general liability.
3. The insured carries a minimum of one million dollars in general liability insurance.

An Agreement to Hold Harmless and Indemnify Form (AHH). This form may be used as proof of insurance for groups with 20 or less participants. AHH’s cannot be used by adult groups. This form must be filled out completely and notarized for the district to accept.

It is the responsibility of the individual who signs the application to read and understand the Regulations For School Facilities or the Guidelines For Use Of District Auditoriums associated with using a Fort Zumwalt School District Facility. This individual should also share this information with all members of the group using the facility.
